Plan Terms and Conditions
- The 20% discount will be reduced to 10% if you use one of our financing options (Sunbit or CareCredit) as your payment method. (The reduction in the discount is due to the fees these companies charge our office for providing their services.) The 20% discount excludes Invisalign, whitening procedures, and implant services.
- This offer cannot be combined with any other offer.
- This is not a dental insurance plan.
- The coverage is valid for one year and the enrollment date serves as the plan’s anniversary.
- Membership fees are non-refundable.
- Appointment times are limited. It is the member’s responsibility to schedule appropriate visits during the membership period.
- There are no refunds for unused appointments (they are non-transferable).
- Excludes care covered under workers’ compensation or auto insurance.
- The savings are valid only at Premier Dental of Woodhaven.
- Services that require the attention of a Specialist are not covered by this plan.
